Dr. Toussaint is a qualified civil surgeon with USCIS. She is committed to helping others complete the immigration medical exam reliably and efficiently.
Immigration Medical Exams
The immigration medical evaluation takes place over two office visits. During the first visit, Dr. Toussaint will review your medical history and perform a physical examination. She will also review your medication list and vaccine records. If you don't have copies of your records, the doctor can order lab work to test for immunity to some vaccines. You will be sent to a commercial laboratory to perform the lab tests (Quest diagnostics). Depending on the lab results, you may be referred for a chest x-ray. Once the test results are available and all outstanding documents have been received, the civil surgeon will complete the I-693 form and provide it to you in a sealed envelope during the follow up appointment. Please ensure that the envelope remains sealed and undamaged. Opened or tampered documents will be rejected by USCIS.

The immigration medical exam is not covered by medical insurance.
You may use an FSA card to pay for the exam, subject to your individual plan’s approval.
The cost of labs will be billed to your medical insurance, if you are insured. Labwork is performed at Quest Diagnostics laboratory. Discounted rates are available for labwork if you are uninsured.
Immunizations may be obtained from your local pharmacy, the NY City immunization clinics or from your primary care provider's office. Many vaccines are offered in our office, at an additional cost.
